How to Apply for a U.S. Passport

Required Documents

  • Application:
    • Unsigned DS-11 form (pdf) completed in black ink. The form must be printed single-sided. It is recommended to fill it out before your appointment. If you do not have access to a printer, we can print out your forms for you. Blank forms are available on-site.
    • *Please note that the DS-11 form is used for first-time applicants or those ineligible to renew by mail.
    • Please make sure to review the additional forms for special situations here.
  • Valid Photo ID
    • Be prepared to present at least 2 types of IDs.
    • Your Social Security Card, although not required, can be used to supplement as an additional form of ID if needed.
    • *Please note that you will have to provide your SSN on the application form.
  • U.S. Citizenship Document
    • Types of acceptable documents include a U.S. birth certificate, a naturalization/ citizenship certificate, or a consular report of birth abroad.
    • If you will be presenting a Texas birth certificate, it is recommended that you submit the long-form version to prevent any processing delays. You can order a certified copy of a Texas birth certificate online or in person. Find more information at the Birth Records webpage of the Texas DSHS.
    • The US Department of State has special requirements for birth certificates from TEXAS, CALIFORNIA, and PUERTO RICO. Please read the following information carefully to avoid a delay in processing your passport application.
Texas

If you will be presenting a Texas birth certificate, it is recommended that you submit the long-form version to prevent any processing delays. Texas issued birth certificates must have a hospital name listed. Long-form birth certificates will have a hospital name listed, however, short-forms do not. Short-form Texas birth certificates are accepted ONLY if there is a letter “i” printed next to the date filed. The US Department of State will not accept short-form birth certificates without the letter “i” which stands for institutional use.

Abstract of Birth: NOT ACCEPTABLE without letter i

You can order a certified copy of a Texas birth certificate online or in person. Find more information at the Birth Records webpage of the Texas DSHS. Alternatively, you can also order a short form version with the letter “I” at nearby county clerk offices and receive it the same day.

California

California issued birth certificates must also have a hospital name listed. Longform birth certificates will have a hospital name listed and are acceptable. California Certified Abstract of Births are generally NOT acceptable by the US Department of State. If you need to order a new California birth certificate, please visit the California Department of Public Health website at https://www.cdph.ca.gov/.

Abstract of Birth: NOT ACCEPTABLE

Puerto Rico

Puerto Rico birth certificates must be issued after July 1, 2010 to be acceptable proof of citizenship as mandated by the Puerto Rico Birth Certificate Law of 2009.

  • One passport photo – we can take these for you at the time of your appointment.
  • Check or money order made payable to the U.S. Department of State. See fees here.
  • Debit or credit card for The University of Texas at Dallas Acceptance Facility execution, photo and copy fees. Scroll down to the Fees section to view our fees.
  • If your legal name has changed, then be prepared to present additional documentation such as a marriage certificate or court documents certifying the legal name change. If you had a passport in the past and your legal name has changed since then, please review the U.S. Department of State website for details on how to update your passport.

Passport Renewals

UT Dallas Passport Services accepts the form DS-11, which can be utilized for first-time applicants, and for applicants who are unable to renew their previously issued passport by mail. To determine if you are eligible to renew your passport by mail or online, please visit the U.S. Department of State. If you are eligible to renew online or by mail but prefer to have someone check over your documents, we are happy to help and can mail in your documents for you if you wish.

Passports for Minors (15 years and younger)

For minors under 16, photo IDs of both parents are required along with proof of parental relationship, such as an original birth certificate. The children and their parents will need to be present. If one of the parents is unable to attend the appointment, additional documentation may be required. For full details and requirements, visit the U.S. Department of State for more information.

Derivative Citizenship

When a parent(s) become naturalized US citizens, their foreign-born children under the age of 18 may become eligible to apply for a U.S. Passport. In order to apply for a passport through this process you are required to provide the following information:

  • Original/Certified Copy of the Foreign birth certificate of the children
  • Certified English translation of foreign birth certificate if not written in English (must be professionally translated)
  • Evidence of lawful permanent residence (such as a permanent resident card or green card)
  • Parent(s) proof of citizenship (such as a Certificate of Naturalization)
  • Parent(s) marriage certificate (if applicable)

University of Texas at Dallas

Execution, Photo, and Copy Fees must be paid by credit or debit card to University of Texas at Dallas Passport Acceptance Facility:

  • Execution Fee per application – $35
  • Passport Photo Print for non-UTD members – $15
  • Passport Photo Print for UTD members- $7 (must show Comet Card)
  • Photocopy Fee- $2 per application
  • Digital Photo – $7 (These will not be cropped to size requirements)
  • Additional Sheet of Photo Prints – $5 each
  • Optional 1-2 Day Outbound priority mail express shipping– Fees can vary based on USPS rates

Frequently Asked Questions

Where is your office located at?

Our office is located at 17217 Waterview Parkway Dallas, TX 75252. Office: ROC 2.902. You will see signs directing to our office once you are on Waterview Parkway.

What are your office hours?

UTD’s Passport Services Office is open from 8am-5pm. Passport services will not be available on university holidays or during university closures. You can view additional closures and updates under our announcements section on our main page- https://oics.utdallas.edu/passport-services/

What documents do I need to bring?

You will need to bring in a proof of citizenship, proof of identity, a credit/debit card, AND a check/money order. For more information on what citizenship and ID is valid please visit the travel.state.gov website

What are the fees and how do I pay? Can I use only a credit/debit card?

We will need two types of payment, a credit/debit card AND a check/money order. Credit/debit card will be used to pay UTD fees. US Department of State fees can only be paid by check or money order. We cannot process the application without a check/money order. Do not include the $35 execution fee on the check, that is a separate payment to our office paid by card.

Does your office offer photo services?

Yes, our office offers photo services. Please do not wear camouflage, government uniforms, and white. Head coverings and accessories are also not acceptable unless they are for religious or medical reasons.

Does your office offer walk-ins?

No, our office does not accept walk-ins. We accept applicants with appointments only. To schedule an appointment please visit our website https://oics.utdallas.edu/passport-services/. You can also call us at 972-883-4715 if you do not see an appointment time that works for you.

What happens to the documents I have sent in with the application?

The original documents you sent in with the application will be returned 2-4 weeks after you have received the passport. If you have not received your documents 4 weeks after please contact the National Passport Processing Center (877) 487-2778

I have a Texas or California short-form birth certificate. What is the difference between the long form and short form birth certificate?

Long-form birth certificates will have a hospital name listed, however, short-forms do not. Short-form Texas birth certificates are accepted ONLY if there is a letter “i” printed next to the date filed. The US Department of State will not accept short-form birth certificates without the letter “i” which stands for institutional use.

Does the applicant have to be present for the appointment even if they are under the age of 16?

All applicants must be present, including minor/infant applicants. We cannot process the application without the applicants present.

What if I am a single parent or have sole custody of my child?

If you have sole custody of your child, please bring the court document showing the agreement. If you are a single parent, you will not need the consent from the other parent.

Do I have to be present for the appointment if my child is 16-17 years old?

No, you do not have to be present for their appointment. Preferably we have at least one parent present for the appointment, if you cannot make it your child will need to present their proof of citizenship and ID. A DS-3053 (parental consent form) can also be filled out by the parents to show consent.

What if the other parent cannot be present?

If the other parent cannot be present, they will need to complete the DS-3053 form and get it notarized. We will need a photocopy of the ID they presented to the notary (ex. driver’s license will need the front & back)

What do I do if I cannot or do not have contact with the other parent?

If you cannot or do not have contact with the other parent, you will need to present court documents showing you have sole custody of your child. If you do not have court documents a DS-5525 (special family circumstances) can be filled out.

My parents are deceased or I do not have contact with them do I still need to add them onto the application form in the parental section?

Yes, it is still recommended that you add them onto the application form even if they are deceased or do not have contact with them to avoid processing delays with your application.

My legal name has changed, what documents do I need to bring?

If your legal name is different then what is on your proof of citizenship document, a court document or marriage certificate may be required. These documents must be original or certified copies (photocopies are not acceptable.)

How long does the appointment take?

Appointments should take no more than 30 minutes to complete. Multiple applicants or special circumstances may take longer.

I have travel plans soon. Could I get my passport faster?

We offer expedited processing that still takes 2-3 weeks to process. If you need your passport sooner than that, go to the Department of State’s Get My Passport Fast page for information on expediting your application, or making an appointment at a passport agency or center.

How do I track the status of my application?

To track the status of your application please visit travel.state.gov. To login you will include your first & last name, date of birth and last four digits of your SSN.
